Please include the following information in your e-mail:

  • In the Subject field of the e-mail -  Full Name and Photo Title
  • In the Body of the email - Title of Photograph, Description (optional), Name, Phone Number, Email, School, Grade (in Fall ’11) and Age.

You must submit a consent form for each person in the photo. Request a consent form from the Belmont Library Information Desk or by email. By entering your photo by email, you give us permission to display or publish your photo online, in print publications or in the library.
